$50 Remote Heart Failure Research Opportunity

Tired of having difficulty accessing your health care data from different organizations? While patients have the right to access and control their own health data, the challenges associated with accessing health data discourage many patients from even trying. This study through Polygon Health and UMass enables you to easily access your own health data and integrate it in one place for simple use and transfer.

Study Overview

Study purpose

To assess if patients gaining comprehensive and easy access to their own health data can improve health literacy (understanding of disease and navigating their care) and have better health outcomes.

Incentives

Free method to control your health by streamlining access to your health data. You will not have to remember different passwords or create new accounts to piece together your vital health records. You can have it in one spot. This method is safe and easy to use.

In addition, we will send you 3 surveys (with 6 short questions) to fill out. One at enrollment, one at 6 months after enrollment and the last at 12 months after enrollment. We will access your health record through Polygon Health to assess and quantify medication utilization and health outcomes. Upon your completion of the surveys, you will be compensated $50 (in Amazon gift cards) for your time and participation. 

Eligibility
Inclusion Criteria
  • Age ≥ 18 years old

  • Have diagnosis of heart failure

  • Have internet access and able to navigate web services

  • Willingness to share healthcare data for research purposes

Exclusion Criteria​

  • ​Have a ventricular assist device or history of heart transplant

  • Have active cancer diagnosis

  • Severe cognitive impairment

  • Unable to consent and complete questionnaire
